Consultation Process

Fill in an "application for consultation" at the hospital, and present it at the reception desk (2) for the first consultation, together with your health insurance card and a letter of introduction (if any).

* Outpatients without a letter of introduction may be charged 2,100 yen (tax included) as a special healthcare expenditure.

Check in at the reception for follow-up consultation (either on the Follow-up Consultation Reception Machine or at the reception desk (1) for follow-up consultation), and wait in the waiting room of your department.

Please understand Make sure that you need to make application at the reception desk (2) for the first consultation if you have not consulted for 7 years or more, or want a consultation in at departments which you have never consulted before.

Pick up your outpatient bill with its holder at the reception desk of your department, and present it to the reception desk (7) for calculation on the third floor.

Your consultation card will be used in commonly and permanently forat all departments. Keep it carefully and make sure to bring it with you for your consultation.

N.B. You will be charged 210 yen (tax included) as actual expense if you need to have the reissuance of consultation card reissued.

1. If you have made any changes in the information on your health insurance card (your name, address, place of employment, etc.), be sure to present the card at the reception desk (1) for follow-up consultation aton your earliest consultation opportunity.
2. If the message of "insurance card confirmation needed" is indicated on the information column of your reception slip output from the Follow-up Consultation Reception Machine, present your insurance card at the reception desk (1) for follow-up consultation before receiving the consultation.
3. If you continue to receive consultation without presenting an insurance card when requested, you may be charged the full cost of yourentire medical charges. on your own.
